Cyst or Lipoma? Here’s How to Tell the Difference

Two of the most common growths we evaluate in dermatology are cysts and lipomas. While they can sometimes feel similar, they are actually very different and may require different treatment approaches.

What Is a Cyst?

A cyst is a sac-like pocket beneath the skin that contains material such as keratin (skin protein), oil, or debris. Many cysts remain stable for years, but others can become irritated or repeatedly inflamed.

Cysts commonly:

  • Feel round or firm
  • Sit just under the surface of the skin
  • Move slightly when touched
  • Sometimes have a visible central pore 
  • Can become inflamed, red, tender, or drain material

Common areas include:

  • Scalp
  • Face
  • Neck
  • Back
  • Chest

What Is a Lipoma?

A lipoma is a benign growth made up of fat cells. 

Unlike cysts, lipomas generally do not drain or become infected.

Lipomas often:

  • Feel soft or rubbery
  • Move easily beneath the skin
  • Grow slowly over time
  • Usually do not hurt

They commonly appear on:

  • Arms
  • Shoulders
  • Back
  • Neck
  • Torso

Do They Need to Be Removed?

Not necessarily. Many cysts and lipomas are harmless and can simply be monitored. If 

removal is needed, treatment depends on the diagnosis and may include observation, medication, drainage, or surgical excision.

Removal may be considered if a growth:

  • Is painful or uncomfortable
  • Repeatedly becomes inflamed
  • Drains or changes appearance
  • Continues to enlarge
  • Interferes with movement, clothing, or daily activities
  • Becomes cosmetically bothersome
  • A definitive diagnosis is uncertain 

Why “Popping It” Usually Makes Things Worse

One of the biggest misconceptions we see is patient’s attempting to squeeze or drain these lesions these at home.

With cysts especially, squeezing may:

  • Increase inflammation
  • Cause infection
  • Lead to scarring
  • Leave the cyst wall behind, increasing the chance it returns

When Should You Get a Lump Checked?

Any new, enlarging, painful, or changing lump deserves an evaluation.

Even though cysts and lipomas are common and typically benign, not every lump under the skin falls into those categories. A professional exam can help determine the next best step.
