What to bring to your first visit
- Photo ID
- Insurance Card
- List of current medications
- List of Allergies
- List of any prescriptions needing refills
- List of your questions for the doctor
All patients under the age of 18 MUST be accompanied by their legal guardian, otherwise they will not be able to be seen
